Care Planning

Introduction

This course is designed to give participants the knowledge of how to develop and implement the care planning system.

Objectives

Participants will learn how to assist in the development of working within the care planning system, in line with the organizations policies. and how to understand and implement the care planning system of practice.

Aims

To give the historical context as to why the care planning system was introduced.
To look at the positive outcomes of using the care planning system.
To look at the potential barriers which may exist to the successful implementation of care planning.
To examine how to overcome barriers which could hamper the implementation of care planning, from individual members of staff, to the team, through to the organizations point of view.
To recognize how implementing the care plans will be beneficial to the clients.

Two days, max 12 participants.
